The best way to indicate your consent
JOIN THE ORGAN AND TISSUE DONATION CONSENT REGISTRY OF THE RÉGIE DE L'ASSURANCE MALADIE DU QUÉBEC (RAMQ).
To ensure that your decision is respected and to make your consent official, register with the Registre des consentements au don d'organes et de tissus(organ and tissue donation consent registry) by simply signing the organ and tissue donation consent form that is sent to you by mail along with your health insurance card renewal notice. You can also contact the RAMQ to order it by phone.
The information sent to the Régie will be kept confidential. When the time comes, Québec-Transplant and Héma-Québec will quickly be able to verify, at the request of a physician or authorized person, if your consent is registered.
Once you have given your consent, you can change your mind at any time. You can obtain an organ and tissue donation consent revocation form from the RAMQ.
Other ways to indicate your consent
SIGN THE STICKER AND PLACE IT ON THE BACK OF YOUR HEALTH INSURANCE CARD.
By signing the sticker and placing it on the back of your health insurance card, you are consenting to organ and tissue donation.
This sticker is sent to you along with your health insurance card at the time of renewal. You can also obtain it from several network health establishments as well as from various organizations that promote organ and tissue donation.
HAVE YOUR DECISION ENTERED IN THE ORGAN AND TISSUE DONATION CONSENT REGISTRY OF THE CHAMBRE DES NOTAIRES DU QUÉBEC.
When you are writing up a will or mandate in case of incapacity, this is a good time to think about organ and tissue donation.
If you consent to organ and tissue donation after freely discussing it with your notary, he or she will indicate this in your will or mandate in case of incapacity. This consent will then be entered in a registry created for this purpose by the Chambre des notaires du Québec.
When the time comes, authorized personnel will be able to verify whether a potential donor has consented to organ and tissue donation, information which is kept confidential.
